RN or Social Work Case Manager - St. Agnes Hospital Care Management (.6)

ST. AGNES HOSPITAL RN Case Manager: Coordinates care to facilitate a safe and timely transition back into the community, alternate level of care and/or with support services. Basic knowledge or willingness to learn computer skills such as word processing and e-mail. Certified Case Manager preferred (CCM, ACM, etc.) CPR certified or obtained within first 90 days of employment. Knowledge of State and Federal regulations for long-term care facilities. Licensed or eligible for licensing in the State of WI as an RN. Prefer minimum of 3 years experience in Case Management. Previous facilitator experience preferred. OR Social Work Case Manager: Responsible for patient care coordination with other members of the multidisciplinary team in order to facilitate a safe and timely transition back into the community, alternate level of care and/or with support services. Certified Case Management (CCM, ACM, etc.) preferred. Current WI Social Worker license is required. Prefer three years Case Manager and facilitation experience. Requires basic computer skills such as word processing and e-mail. Requires knowledge of State and Federal regulations related to utilization review and discharge planning.
